How to request Awards:
- Click on the Award title to bring up the criteria and application form for that specific award.
- Select the method you wish to use to purchase the Awards:
- Use the online form. Complete one form for each recipient Once the form is submitted, you can pay online via PayPal. You may use your PayPal account or a credit card for payment.
- Download a form. Click the download button to access the printable version of the application form. Complete the form and mail it with a cheque or credit card information to:

IMPORTANT: Certificates should be ordered at least Four (4) weeks prior to
presentation to insure delivery in time for your event.
The following awards are provided by OAAS for Agricultural Societies to present to deserving recipients for service to their local Agricultural Societies.
Meritorious Service Award <click on this link to bring up Award criteria and application form>
Each Agricultural Society is allowed to present a maximum of ONE (1) ‘Meritorious Service Award’ per year
Cost per certificate: $10.00 plus HST.
Years of Service Award <click on this link to bring up Award criteria and application form>
Each Agricultural Society may present as many ‘Years of Service Awards’ per year as it wishes to recognize members for specific Milestone or long-term years of service to its society.
Cost per certificate: $10.00 plus HST
Accomplishment Award <click on this link to bring up Award criteria and application form>
Each Agricultural Society may present a maximum of FIVE (5) ‘Accomplishment Awards’ per year to recognize service or contribution to its local Agricultural Society.
Cost per certificate: $10.00 plus HST

Priority Post - $13.27 additional cost (plus HST) – Canada Post guarantees delivery in 2 to 3 business days.

NOTE Multiple certificates ($10 each) on the same order will be shipped for the same Priority Post fee. (Can include ‘Meritorious Service’, ‘Accomplishment’ and ‘Years of Service’ certificates in the same package for same price).
Ordering Multiple Certificates online
An Agricultural Society can order multiple Awards and/or select Priority Post shipping option in one PayPal or credit card transaction by following these steps. This order may contain any number or combination of Award applicatons.
- Complete first application form and click on the ‘Add to Cart’ button - this will bring you to the PayPal screen.
- DO NOT complete the Payment option but click on the 'Continue Shopping' link at the top right corner of the PayPal screen - this will bring you back to the same form so you can complete another application.
- If you wish to complete the application form for one of the other awards, go to the bottom of this application form and select the "Return to Main Awards Page" link. You can select the application form for one of the other awards.
- Click on the next application form and complete it – click on “Add to Cart’ button - this will bring you back to the PayPal screen again. Continue to complete the forms as required – DO NOT complete the Payment option until you have completed filling out all the application forms.
NOTE: if you want to pay to have your Certificates and pins sent by Priority Post – select "Add to Cart" button beside the Priority Post option at the bottom of the application form at this time.
- Once you have completed the last application form, you will be brought back to the PayPal screen.
- Complete your PayPal payment transaction. You may use your PayPal account or a credit card for payment.
